"In my former role as Head of Major Crime Operations at Grampian Police, I had sole jurisdictional responsibility for overseeing all investigations into North Sea offshore incidents. Often multi-fatality tragedies, their common denominator was they were all preventable. In identfying the chain of events that led to the cause of these major incidents, it was easy to determine where human error and/or equipment failure occurred and hence where the incident could have been prevented. Building on this unique experience, I became passionate about bringing this to the Corporate Sector to help improve safety and human performance. At Matrix, we work on the basis that blame is useless. We help you look beyond WHAT happened in an incident or system failure, to focus on WHY it happened. Then you will be far better placed to implement corrective action and prevent a reccurrence. That's why we approach incident investigations as an organisational opportunity as opposed to an organisational threat!"

Alan Smith Former Detective Superintendent and Founding Director of MRC.
