    Our Team

    Matrix Risk Control

Specialists in all aspects of the investigation and analysis of safety, environmental, quality and equipment failure incidents. Our experienced team deliver a range of training and support services designed to assist organisations

    Alan Smith

Alan is a former Detective Superintendent. With the portfolio for Major Crime Operations, he held the strategic lead within his Force for all Homicide and Kidnap Investigations. A unique, additional role was leading investigations into high profile major incidents within the North Sea Oil and Gas Sector. It was during these investigations that Alan identified a common skills gap in the ability of major organisations to perform basic incident investigation and analysis and thus understand why they suffer repeat failures.
In 2008, with the objective of plugging this skills gap, Alan co-founded Matrix Risk Control with their focus on helping organisations through both training and direct support to perform effective and professional incident investigations. Alan has built extensive personal experience as a Root Cause Analysis instructor globally and is often invited to deliver case study presentations from his past experience including the landmark 1988 Piper Alpha disaster where 167 men lost their lives.

In 2004 he graduated from the Executive Leadership Programme at the FBI National Academy in Quantico and from the Atlantic cohort of the International Leadership in Counter Terrorism programme in Montreal in 2007. On leaving the Police in 2009 he accepted a consultancy role with the Scottish Government leading a national criminal justice project, remaining in post until September when 2011 he joined MRC to develop and grow Incident Investigation Training and Support and expand into new markets. Ian and his colleagues developed the Matrix Incident Investigation Model for industry, based on tried and tested investigative techniques and their personal experience performing incident investigations in the Oil & Gas, Engineering, Maritime, Transportation, Heavy Industry, Utilities and Aerospace sectors. The training has since been successfully delivered to hundreds of students across Europe, the Middle East, North America and Australia.
